KEY OBJECTIVES:

To provide high quality Occupational Therapy services inclusive of clinical supervision, support and assessment for the Fusion CHSP Program.

KEY RESPONSIBILITIES/TASKS:

Provide assessment, treatment, and referral for all FBM eligible clients referred in accordance with local and professional guidelines and protocols

Assess the need for home modifications, aids and equipment.

Support the goals of the Fusion by: - Assisting clients to live in their community as independently and autonomously as possible - Ensuring clients are actively involved in making decisions about their life, what services they receive and the goals they wish to achieve - Working collaboratively with FBM clients focusing on promoting capacity building and restorative care

Fusion Australia Ltd, is a Christian not-for-profit youth and community organisation.  Services provided include creative youth services, Occupational therapy services, frail aged and disabled home modification services & equipment prescription, building contracting service and a variety of community activities.

Fusion Building & Maintenance,Home Modifications and Maintenance programs are funded through the Commonwealth home support program (CHSP), These programs provide home modification and maintenance services to the homes of frail aged. Fusion is also a provider of services under the National Disability Insurance Scheme.

Qualifications:
  • Batchelor of Occupational Therapy
  • AHPRA registration
Essential Selection Criteria:

Essential Criteria:

  • Tertiary qualifications in Occupational therapy
  • AHPRA registration.
  • Excellent communication and interpersonal skills.
  • High level of professionalism and attention to detail.
  • Computer and technology skills.
  • Provide assessment and OT Reports for all CHSP and NDIS eligible clients referred, in accordance with professional guidelines and protocols.
  • Demonstrated ability to assess the need for home modifications and maintenance & equipment.
  • Successful clearance of Working with Children and Criminal Record checks, from relevant Departments.
  • Willingness to work within a framework of Christian values.
  • Driver’s Licence.
Desirable Selection Criteria:

Desirable Criteria:

  • Experience with CHSP or similar government funded services.
  • Experience with the needs of frail aged and people with a disability.
  • Administrative experience in small business arena.
  • Awareness of child and elderly protection issues.
  • Basic knowledge of building terminology
  • Experience in the Not-for-profit sector. 
Other Information:

This position is for 38hrs a week, but is negotible if required
