The Role: 

The Tripoli & Mena Association Aged Care is seeking a Home Care Package Case Manager to assist existing clients and manage new recipients, in the inner west and western suburbs of Sydney. The Home Care Manager supports clients through the whole life cycle of a HCP, including intake, onboarding, risk and needs assessments, development of goal-directed care plans, coordination of services and supports, referrals, regular reviews, and discharge. The Home Care Manager also administers the financial, budgetary, contractual and compliance aspects of the Home Care Package Program in accordance with the Aged Care Quality Standards, funding, and legislative requirements. You will work collaboratively with other team members to ensure that our program is best practice and meets the Aged Care Quality Standards, while ensuring that client service, processes and practices are client focused and aligned to company values and goals, policies and procedures and other legislative requirements. The role reports to the Home Care Package Manager.

Qualifications

Australian Tertiary qualifications in nursing, allied health, human services, or other related field, or lesser formal qualifications with substantial experience in working with elderly people or people with disabilities in a home and/or community setting

Essential Criteria

  • Strong knowledge of Home Care Packages program and well-developed skills in providing assessment, person-centred goal planning, service coordination and case management in community aged care
  • Excellent communication, planning, and problem-solving skills
  • Fluent in English & Arabic

Desirable Criteria

  • Demonstrated awareness of issues faced by older people in a home and/or community setting and ability to develop positive and productive relationships with clients.
  • Experience in liaising, consulting and negotiating with other service providers as well medical, allied health and other professionals to achieve best outcomes for a client.
  • Demonstrated experience in client budget planning and monitoring in a context of HCP program.
