We are seeking a values-driven and dedicated Case Worker, reporting to the Manager. You will provide effective service delivery, ensuring case management, assessment and referral, education and support are provided in accordance with TSA guidelines.
This is a permanent full-time position based at our Dooralong Transformation Centre.
You will successfully
- Provide efficient day to day service delivery through education, interventions, group work, assessment, referral and case work
Implement and monitor the development of client-centred collaborative case plans, including exit planning
Supervise the self-administration of medication as required and record as per TSA policy and procedure
Identify, assess and manage participants risks and refer to the line manager as required
Demonstrate commitment to an environment that values continuous improvement and innovation
The Salvation Army is subject to public health orders. You may be required to provide evidence of vaccine status dependent on requirements.
You will have
- Minimum of Diploma in a relevant discipline
- Knowledge of AOD/Gambling, Community Services and Health Sectors
- Demonstrated case management experience
- Experience facilitating psychoeducation and therapeutic groups to diverse populations
- Experience working with people from Aboriginal and Torres Strait Islander backgrounds and from culturally and linguistically diverse backgrounds and religions
- A demonstrated ability to deal with problems associated with substance abuse, gambling and mental health conditions
- A valid Employee Working with Children Check
- A valid NSW Drivers licence
- A willingness to undergo a National Police Check
