The core mission of Anglicare (NSW South, West & ACT) is to break the cycle of intergenerational poverty for children, young people and families. Your role as a Case Manager is integral to this mission, as we mainly focus on serving vulnerable communities in the ACT and regional, rural NSW.

We seek individuals who share our vision and values and are eager to join our community as the newest Case Manager for the Permanency Support Program—Foster Care & Kinship Care.

Key responsibilities:

Manage all aspects of case management within the Permanency Support Program (Out of Home Care) service.
Ensure the safety, well-being, and positive, permanent outcomes for young people.
collaborate with families, carers, and other stakeholders to support and empower children and young people.
Maintain thorough and accurate documentation to support case plans and outcomes.
advocate for the needs and best interests of young people within the program.
About you:

Your child and family sector expertise and experience working with Aboriginal communities will be invaluable. Top-level communication skills and cultural competence are essential. A minimum diploma qualification in Community Services or a related discipline is required.

You will have the autonomy to make decisions while respecting the trusting relationships with our service and the families and children you support.
