Case Manager - Mental Health

We are searching for an experienced Case Manager to support the Personal Helpers and Mentors Program (PhaMs). You will support participants who identify as having a mental illness to apply for and access the NDIS for support. The role will also allow for the Case Manager to deliver support co-ordination under the NDIS to those eligible participants, as well as NDIS participants who wish to access the PHaMS program for their support co-ordination. For participants not eligible for the NDIS, you will offer a time limited period of case management support, by assisting participants by providing direct case work through mentoring, psychoeducation, assisting with referrals, and linking participants with appropriate services. Support will be strengths-based and recovery-focused.


This is a full-time opportunity until 30 June 2019, and includes access to salary packaging.

Position details

Tertiary qualifications in Social Work, Psychology or a relevant social welfare qualification.


Essential Selection Criteria:
  • Demonstrated experience working with people with mental illness, demonstrated knowledge of mental health issues, and practical experience in case management
  • Strong time management skills, together with the capacity to work independently and collaboratively in a team environment meeting competing priorities and time frames
  • Significant understanding of how to support clients to access the NDIS and deliver support coordination under the NDIS
  • Highly developed communication and problem solving skills including crisis intervention
